The Foundation’s Grant Agreement requests these important provisions be followed:
- A written report on the disposition of grant funds received and the status or outcome of the project funded must be submitted by April 15 of the following year. Please use the Grant Report Form template provided here. Failure to submit a written report and pictures may disqualify you for future grants.
- Submit the Grant Report along with 1-3 pictures or a short video to firstname.lastname@example.org
