Organizations that received funding from the Foundation in 2020 must submit a brief, written Grant Report detailing the use of funds by the application deadline in order for a new application to be considered - even if prior year grant funds have not been fully expended. A Grant Report template is available on our website. (See Main Menu: For Ministries/Grant Reporting). If you received a grant last year, please upload a copy of your Grant Report in Part III "Required Supporting Materials". (Note: If you received a grant in an earlier year and failed to submit the required report, you may submit a late report in Part III along with an explanation for the oversight.)

Your request cannot be considered without providing the following supporting information to the Foundation. All supporting materials are due April 15. If April 15 falls on a weekend or holiday, the due date is extended to the next business day. PLEASE SUBMIT ALL SUPPORTING DOCUMENTS IN A PDF FORMAT.
1. Narrative Summary of the Grant Request
Please provide a narrative description of your grant request (3 pages maximum). Include a brief history and description of your organization; a description of the need or problem addressed by your grant request, the persons that will be served, plans for funding, and timing for the project. The Foundation strongly prefers projects that will be completed within one year of funding.

2. Current Year Operating Budget
A copy of the organization’s current year operating budget with revenue and expenses. (Annual, condensed budget, 1-3 pages, is preferred.)

3. Financial Statements for the most recent fiscal year
Please submit a Statement of Net Assets (aka Balance Sheet) and a Statement of Activities (aka Profit & Loss) for your most recently completed fiscal year. Do not submit partial year statements. Audited financial statements are preferred but not required. If you have an annual audit but it is not completed for your most recently completed fiscal year, submit in-house financial statements for the most recent fiscal year AND your most recent audited financial statements available. (Note: If your organization has a separate supporting foundation or endowment, please provide financial statements for that organization as well.)

4. Detailed Budget for the Grant request

5. Tax Exempt Letter
A copy of your most recent IRS Determination Letter indicating your agency’s non-profit tax-exempt status. If your church, school, or agency falls under the "group tax exemption" of the Episcopal Church, you should be able to provide a letter from the national church Treasurer as documentation.
